Field trip to Perot Museum

Assalaamu alaykum!

The elementary students at MSE are going on a field trip to the Perot Museum of Nature and Science. It is sure to be a day of fun, learning, and excitement InshaAllah. After viewing some exhibits students will enjoy watching the movie “Wild Africa,” and will have lunch at the Museum’s Plaza area before heading back to the school.

Please read the information at the top of this form, then sign and return the permission slip at the bottom of the form emailed to you along with the $15 admission fee by Wednesday, 4/5/17.

Please note that students will not be able to participate without a signed permission slip.

Where: 2201 N. Field Street, Dallas, TX 75201

When: Thursday, April 6th , 2016

Depart school: 9:00 am

Estimated Arrival back at school: 2:30 pm

Transportation : Students will be transported by school bus

Admission and Bus Fee: $15 (cash)

Items for your child:

 $15 fee and permission slip in an envelope (returned by 04/04/17)

 Jacket; weather appropriate clothing

 Comfortable, closed- toe walking shoes

 Water bottle + lunch stored in a disposable bag labeled with your child’s name

 Small backpack or shoulder bag that can carry the water bottle and lunch
